SBAOA’s Community Health Worker (CHW) Program provides free support for eligible Medi-Cal members who need help navigating healthcare and community services. Our Community Health Workers can help you schedule medical appointments, connect with primary care providers and specialists, access behavioral health and dental services, understand your health benefits, and find community resources that support your overall well-being. We provide personalized, culturally responsive guidance to help you overcome barriers to care and stay connected to the services you need.
